新生體檢
Health Check-up for 2020 Fall Semester Students

由於體檢項目不一，即便有做過簽證體檢的新生，請務必做新生體檢。
Health Checkup is a required process for every NPTU new student. Due to the different inspection items and contents, those who did a medical examination for visa application before are also required to have Health check-up for new students.

流程
Procedure

Step1.至線上健康資料填寫並拍照截圖至手機 (登錄操作方式請見附件)。
Fill in online health information and take a picture of the screen to your phone. (Instruction for how to fill in, please see the attached)

Step2.至臺銀網頁列印繳費(可至超商或郵局等方式繳費)
Go to the online website to print out the bill and pay.(You can go to the convenience store or the post office to pay the fee)

Step3.依規定時間到校健康檢查
Go to school and do the medical examination by time

時間地點
Time and Location

已在臺外籍新生：依班級系所時間一同健檢。（時間地點請見附件）
Students who are already in Taiwan：follow the schedule by class (see the attached)

8/20新入境外籍新生：於9月11日早上7點15分於屏商校區學生活動中心集合健檢。
Students who arrived in Taiwan on 8/20：September 11th, 7:15 A.M. at Pingshang Campus (Student Center)  Pingshang Campus

8/27(含)以後入境外籍新生：將另安排時間健檢。（等候國學組通知）
Students who entered Taiwan on 8/27 and after：arrange another time for health check-up (informed by OIA)

(1)繳費收據
Payment receipt
※請將繳款單與收據訂在一起並妥善保存 (please staple the payment bill and the recept together and keep )

(2)一吋照片
One inch photo

注意事項
Notice

！檢查前 6 小時必需禁食任何食物
Do not eat and drink for 6 hours before you do the health check.

！請穿著素面上(內)衣，不要有金屬拉鍊、鈕扣，勿戴項鍊，以免影響 X 光檢查結果。懷孕中暫不照胸部 X 光。
Ｗear plain (inner) clothing. No metal zippers, buttons, or necklaces to avoid affecting the X-ray examination results. The chest X-rays will not be taken during pregnancy.

！生理期恐將影響尿液檢查結果，請務必告知驗尿人員
It may be affected the results of urine test during menstrual period. Please inform the urine test staff in advance.

檢查報告由健康中心驗收後於 11 月發給同學
The medical examination report will be sent to students in November by NPTU Health Center.
