本表將與其他審查資料一同提供給系所，以協助確認是否開課或通知是否需改選課程。
*請注意即便列於上學年開課表的課程，也有可能下學期未開課。
請於下方網址查詢課程資訊，步驟請參考“課程資訊查詢步驟＂。

This form should be uploaded on the online application system.
This form will be sent out to departments to confirm whether the course will be open for incoming semester, or to inform the students to select another course.
*Please note that even courses listed in the previous academic year may not be available for next semester.
